 AFFIADVIT

 

          I, Zeeshana Kanwal  widow of  Abdul Raheem Rank: Major, Department: Pak Army, do hereby solemnly affirm and declare that my husband died on 03^rd April, 1997. Photocopy of death certificate of my husband is enclosed herewith). I am drawing family pension (the photocopy of pension book is also enclosed). I further declare that I am owner of the House No. 644, Gali No. 8, Mohallah Chaklala Scheme III, Rawalpindi, in the cantt jurisdiction of Cantonment Board,  Rawalpindi. There is no other co-owner . I also declare that I have not applied for 100% Exemption envisaged under the above  gazette notification, in any other Cantonment in the country. The 100% exemption applied for the period w.e.f _________to ___________

 

          It is further declare that the contents of above affidavit are true and correct to the best of my knowledge, belief and nothing has been concealed thereform.
