Format of Application for Entrustment of case due to Abolition of Court
If earlier, you had filed any civil/family case and same was decided in your favour and now you want further order in same case from same court but the court which decided your case has been abolished, you can file an application to District & Sessions judge of that district for entrustment of your application to any other court for deciding. Following is the sample of application

2. That on the above said application, the applicant was appointed Guardian of person of the said minor vide Guardianship Certificate dated 19-05-2017 by the Court of Mr. Zubair Nabeel, learned Guardian Judge, Rawalpindi
3. That the applicant wants to apply and obtain/ receive passport of above said minor from the Passport Office, and want to take the minor abroad.

That the court of Mr. Zubair Nabeel, learned Guardian Judge, Rawalpindi has been abolished, due which the applicant is unable to file the application for permission to apply for passport of minor etc before the court, which issued the guardianship certificate of the minor, hence this application for appropriate order.
