Format of Affidavit for Child’s Domicile Certificate – Affidavit by father for son

AFFIDAVIT

 I, Tayyab Mehmood Alam Butt of Nabeel Aslam Mehmood Khan resident of House No. 199, Street No. 54-C, Mohallah Cantt Chiragh, Rawalpindi do hereby solemnly affirm and declare as under: -

 

1.               That Mr. Atif Ahmed Nabeel is my real son.

2.               That the facts stated in attached application for the grant of domicile certificate for my son are true and correct.

3.               That my above said son has been residing in District Rawalpindi since birth.

4.               That before this we did not apply for said certificate from any other district of Pakistan.

5.     That we did not migrate from any other foreign country.

I further declare that the contents of my above affidavit are true and correct to the best of my knowledge and belief and nothing has been concealed there from.
